TELEHEALTH COVERAGE

VERIFICATION REQUIRED

Verify your Telehealth Coverage prior to your appointment.

INSURANCE
  • ​​Please be sure to verify your Behavioral Health Benefits with your insurance plan. 

Failure to do so may leave you financially responsible for unexpected insurance deductible expenses or denied claims  due to not having out of network coverage.

  • You should know your copayment and deductible amounts.

  • We are not In Network with all plans.

  • Verify the provider's location is "Life Management for Adults". 

Our providers are In Network with some insurance plans at Portsmouth Hospital that we do not participate with at  Life Management for Adults. 

If our provider is showing In Network with your plan at Portsmouth Hospital but NOT at Life Management For Adults then you will not be covered to be seen at this practice.

HOSPICE - Insurance will not cover patients once they are on hospice. The patient will be financially responsible at the Self Pay rates. 

FINANCIAL PSYCHIATRIC FEES  (if the client does not have insurance)

Initial Psychiatric Evaluation, 45-60 minutes:                            $400.00 - $420.00

Follow up, for medication management only                                 $170.00 - $180.00

Follow up for medication management and psychotherapy       $220.00 - $230.00

and provide copies of updated Insurance Cards Prior to Each Appointment

Call the office to update your information over the phone or

use the Submit Updated Insurance Form

 

We need a copy of the front and back of your insurance card.

Telehealth Coveage

You are responsible for verifying your Telehealth Coverage prior to your scheduled appointment.  

Although some insurance companies will continue to cover telehealth sessions, this does not guarantee this benefit under your individual plan. 

 

We are not able to confirm this coverage for you as each members benefits vary.

If you have already verified that your Insurance Plan covers Telehealth for our Provider at this location (Life Management for Adults),  please complete the verification form.
